Registration
Symposium Registration Fees
Member and Non-member registration fees include the Symposium Proceedings,
access to scientific sessions, exhibit hall, daily continental breakfast,
morning and afternoon coffee breaks during the scientific sessions, and
admission for one to the Welcome Reception and the President's Gala Reception.
(Lunch is not provided.)
A separate registration fee must be paid to attend the Continuing
Education course(s). The NTP satellite Symposium
is free, but registration is required.
Spouse/Guest Registration
The Spouse/Guest registration fee includes continental breakfast (Monday
through Thursday) in the STP Guest Hospitality Suite at the Fairmont Hotel
Vancouver, one admission to the Welcome Reception Sunday night and the
President’s Gala Reception Tuesday evening.
Student Registration
The Student registration fee includes the access to all sessions and
exhibits, daily continental breakfast, admission for one to the Welcome
Reception, Student Reception and the President's Gala Reception. A letter
of verification from a department chairman MUST accompany student registrations.
Any student registration received with no verification letter will be
processed at the higher NON-MEMBER fee. Copies of student identification
cards are NOT acceptable.
Symposium Registration Instructions
Register On-line — closed (Register by fax until Thursday June
15 or on-site after June 15)
Alternatively, Registrants paying by check or those not utilizing the
Internet may submit their Registration Form by fax or mail.

Forms will be date-stamped as they arrive. This is your date of
registration. Faxes will be accepted by STP Headquarters only if
a credit card
number
is clearly listed in the appropriate area.
NOTE: To prevent double-billing, if you are registering
by fax, DO NOT mail your original registration form. STP needs
only one
copy for processing. All mailed and faxed registration forms
will be processed on-line by STP staff.
Cancellation Policy
If you must cancel your registration, all requests must be received
in writing to Jessi Canning at STP
Headquarters no
later than May 6, 2006. All fees will be refunded less a $25 processing
fee.
